Consent for Personal Information

Please read and complete the following form regarding the processing of personal information by the NDAEB for purposes related to the assessment of your application and administration of examinations.

I understand that for the purpose of assessing my application to register for and participate in the Written Examination and/or Clinical Practice Evaluation (CPE), the NDAEB collects personal information including: my name, photograph (for identification purposes at the examination facility), mailing address, educational transcripts/diplomas or letter(s) from my school confirming my enrollment in or graduation from an eligible dental assisting educational program.

I understand that if I received my dental assisting (or allied) education outside Canada, the required documents will also include, but may not be limited to, report(s) from third parties such as translators and international credentials evaluation agencies confirming my academic records.

I understand that if I request special exam accommodations for medical or other reasons, I may be required to provide the NDAEB with reports or other documents from third parties such as, educational counselors, psychologists or physicians to support my request.

I understand that if I request a special exam sitting date for religious reasons, I may be required to provide written confirmation from a religious leader before the NDAEB considers my request.

I have read the NDAEB Privacy Policy regarding the collection, use, disclosure and protection of my personal information and I have been provided with contact information and a telephone number should I have questions or concerns. The NDAEB Privacy Policy, which governs the NDAEB’s practices with respect to the collection, use and disclosure of personal information, is readily available at https://ndaeb.ca/privacy-policy/.

As indicated in the NDAEB Privacy Policy, I understand that I may request access to my personal information and request that corrections be made. I may also refuse to provide some personal information or request to withdraw my consent; however, some personal information is necessary to fulfill the NDAEB’s mandate. As such, the NDAEB may be unable to provide its services if necessary personal information is not provided.

I understand that, as part of the mandate and in the interest of public safety, the NDAEB may disclose personal information, which may include my name, my pass/fail result(s) of the NDAEB Written Examination and/or Clinical Practice Evaluation (CPE), my NDAEB Certificate Number, the date of my examination sitting, the date of my taking the CPE, and the date of the completion of my Transfer of Credentials (ToC)to the Canadian Dental Assisting Regulatory Authority (CDARA) in the province in which my address is listed, the province in which I wrote the exam and/or took the CPE, to any other CDARA requesting information about me, and to any other CDARA to which the NDAEB considers, in its sole discretion, that such disclosure is appropriate.

I understand that the NDAEB may retain my personal information as long as is necessary to meet the requirements of the NDAEB mandate. I hereby give my consent to the NDAEB to collect, use, disclose and protect my personal information as set out above.

You have selected that you do not consent to the processing of your personal information by the NDAEB.

As a result, the NDAEB is unable to continue with your registration. You may wish to review the Privacy Policy and/or the above Consent for Personal Information again. If you do not wish to proceed, you may close your application.

Personal Information

The NDAEB certificate is an official document used in the registration/licensure of dental assistants in Canada. Your name will appear on your NDAEB certificate EXACTLY as typed below. Use only your official given name(s) and surname as they appear on your valid photo identification that you will present on the day of the exam. Do not use nicknames. If the name appearing on your academic records is different from your name as stated below, you will be asked to provide proof of name change (ex. marriage certificate).

Eligibility Self-Assessment

If your dental assisting program or dental health program is not listed on the Dental Assisting Schools page of the NDAEB's website OR you are not a recent graduate from a dental assisting program, you MAY NOT be eligible for the written examination at this time without a program assessment and/or completion of a recognized dental assisting upgrade program. Please complete the following fields to self-assess potential eligibility for the NDAEB's written examination. If you have completed formal dental assisting or dental health training through more than one program, please complete the following fields based on the program where you received the MAJORITY of your formal dental assisting or dental health training.

If you have completed your dental assisting program but have not yet received your final diploma or official transcript, please submit a Letter of Completion from your dental assisting school. The Letter of Completion must:

• Be on official school letterhead and signed by a designated faculty member
• Include your full name as it appears on your application
• Confirm your program start and completion date

Additional Dental Assisting/Dental Health Program(s)

If you have completed your dental assisting program but have not yet received your final diploma or official transcript, please submit a Letter of Completion from your dental assisting school. The Letter of Completion must:

• Be on official school letterhead and signed by a designated faculty member

• Confirm your program start and completion date

Dental Assisting or Dental Health Program Proof of Completion

In this section, you will be required to upload your dental health program proof of completion document(s).

Exam Session Details

You may apply for the March, June, September or December 2025 examination session. Please refer to the Written Exam Schedule on the NDAEB website for applicable deadlines for each exam session. Please select your preferred examination session, language, and location below.

Request for Special Testing Accommodations

The National Dental Assisting Examining Board may provide special testing accommodations based on religious reasons or medical and/or pregnancy/maternity related needs.

The NDAEB will assess each request for test accommodation on a case-by-case basis. The decision to grant an accommodation and the type of accommodation granted is at the sole discretion of the NDAEB. The decision and type of accommodation granted, if any, will depend on the nature and extent of your personal situation, the accommodation requested, the supporting documentation provided, and the setting, context and requirements of the specific examination.

If the NDAEB decides to grant an accommodation, it will be valid only for one examination session. Separate accommodation requests must be made each time an accommodation is required for an examination. Note that all types of accommodations may not be available at every site or on every date. The NDAEB reserves the right to determine the date, location and time of examination sessions.

I acknowledge that, all supporting documentation pertaining to my selection(s) of the Request for Special Testing Accommodations is included in this application form and that any outstanding supporting documents will be sent to the NDAEB by the deadline date or my request for special testing accommodations will not be granted.

I understand that the NDAEB reserves the right to determine whether my Request for Special Testing Accommodations will be granted and that I will be advised in writing of the decision.

Voluntary Disclosure of Personal Information to DA Association or Other Third Party (Other than a CDARA)

With your voluntary consent, the NDAEB will release your name, NDAEB certificate number and date of exam or CPE sitting to a dental assisting association or other third party (one only).

Should you wish to have the NDAEB voluntarily release your name, NDAEB certificate number and date of examination or CPE to a provincial DA association, or third party other than a CDARA, you must read and voluntarily sign the consent form below. Releasing your name and NDAEB certificate number does not mean that you are automatically licensed/registered/certified in a province. You must contact the CDARA or DA association for details regarding provincial licensure/registration requirements.

Please note, in the event you are unsuccessful on your exam or CPE, the NDAEB will not release your information to a third party, EXCEPT FOR A CDARA, even if you have signed this consent form.

You may voluntarily disclose your NDAEB information, directly to a third party without the knowledge of the NDAEB. Please retain your exam and CPE results and NDAEB certificate in a safe place for future use.

You are not required to sign this form when registering for the exam or CPE. Should you wish to have the NDAEB disclose information on your behalf at a later date, you may request a form at some time in future.

Note: in the interest of public safety, the NDAEB will automatically release your exam or CPE results, pass or fail, to the Canadian Dental Assisting Regulatory Authority in the province in which you wrote the exam or took the Clinical Practice Evaluation (CPE) or to another provincial CDARA upon request of the CDARA. Please refer to the written exam or CPE application form and NDAEB privacy policy for details.
