Certificate Reprint Request

If you have you lost your original NDAEB paper certificate or wish to request a reissuance of your NDAEB certificate because of a name change, you can now apply for a new paper and digital NDAEB certificate using the online NDAEB Certificate Reprint Application. Please be advised that the certificate reprint fee and credit card processing fee are non-refundable.

Returned NDAEB Certificate

If your NDAEB certificate is returned to our office and it is determined that your address on file was invalid or you did not maintain your current address with us, your NDAEB Certificate will be destroyed. Should you wish to order a reprint, you will need to complete the NDAEB Certificate Reprint Application and the full certificate reprint fee will apply. Please be advised that the certificate reprint fee and credit card processing fee are non-refundable.
